The IHSAA Athletic Physical form is also available at the bottom of this page. Athletes must have an athletic physical on file dated after April 1, 2023 to be able to participate in practices/tryouts. • All athletes will register through FinalForms. • Parents will electronically sign off on all the participation forms. • IHSAA Physical paperwork may be uploaded to FinalForms or submitted to the athletic office. • Once all sections of the online registration are completed by the athlete and guardian(s), the student's name will become highlighted in green. Student-Athletes must be highlighted green in FinalForms to participate in the first official day of tryouts/practice.
