Physical Turn In Nights for Online Athletic Registration

We are excited to announce that the Western Boone Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your students, saving you time.  You may review your data at any time to verify it is current.  You will be required to sign your forms once per year and after any update.

We are asking that ALL parents of athletes (5-12) to use FinalForms for athletic registration.  Please keep in mind that we are not eliminating any of the items that need to be processed for your child to participate in sports at the Elementary, Junior High or High School.  We are going away from the “Star Card” and going paperless, making the process of participating in sports and communicating during the sports seasons much easier.

Western Boone Final Forms Parent Playbook

In previous years all participating athletes at Western Boone have been required to turn in their participation packet forms to the athletic department and they were issued a “Star Card” to identify them as eligible to participate in athletics.

We have used Summer Star Card Nights as a way to expedite that turn in process.  As we now transition away from “Star Cards” to having ALL athletes completing their athletic registration online, we will utilize the previously announced “Star Card” nights as “Physical Turn In Nights”, as well as having computers available to complete online registration on site where we can answer any questions a parent or athlete might have while creating their account and registering.

The reason for the need to have “Physical Turn In Nights” is that this is the only hard copy form that will need to be turned into the Athletic Department from this point forward for completion of the athletic registration.

Parents and Athletes will now be able to do the following;

  • Create accounts on FinalForms
  • Enter emergency medical information and history
  • Identify sports athletes plan to participate in
  • Sign off on all participation forms electronically (except for the physical)
  • Print out the part of the physical (pg. 2 & 4) that requires parent and physician signatures and turn those into the athletic department.
  • Keep up to date on the latest communications from the athletic department and coaches

Once that IHSAA physical form is turned into the Athletic Department, the Athletic Department will confirm receipt of the form on FinalForms and as long as all other documents are electronically signed and information is entered correctly, the student-athlete’s name will turn green in the system and that green label will identify the athlete as eligible to participate.  If any one part of the process is not complete, the student-athlete’s name will remain red and the athletic department and coaches will use the final forms communication system to notify parents and athletes of what is needed.

FinalForms will now allow the athletic department as well as all coaches to directly communicate to specific parents, teams, season, etc through the email address that is provided during the creation of the account and registration.  This will streamline our communication to parents and allow for one uniformed communication device form the athletic department and coaches to parents/guardians.

The Summer Dates where the Athletic Department will be open and accepting signed copies of the IHSAA Athletic Physical, as well as providing computers to parents and athletes who have not created their FinalForms account and registration are as follows:

July 11th, 2018

HS Football Team- 4:00 p.m.- 6:00 p.m.

All other sports- 6:00 p.m – 8:00 p.m.

July 18th, 2018

All Sports: 6:00 p.m – 8:00 p.m.

*If you have a physical that has been completed after April 1st, you may turn that physical in on any day that the athletic office is open.
